I.M. Well Medical is Transitioning to Direct Pay
We at I.M. Well Medical would like to share an important update about the new upcoming chapter in our medical practice. Starting September 1, 2025, our medical office will be transitioning to a direct pay model. This means, I.M. Well Medical will no longer be accepting insurance and costs will need to be covered out-of-pocket.
Why the change?
This pivot in our care model was a difficult consideration. The unfortunate reality is that insurance requirements pose a heavy threat to the kind of healthcare service we believe is deserved for all patients including longer appointment times, communication via email and text, and treatment decision making.
Rather than closing our doors, I.M. Well Medical is choosing to act in accordance with our commitment to providing the highest quality care to our patients.
Are there Pay-per-Service Options?
Yes, I.M. Well Medical will be offering pay-per-service options! Price listing is as follows:
$575 | Annual Exam
$475 | Follow Up Appointment
$150 | Telehealth
$750 | House Call
$575 | Same Day Urgent Care

Can I get Reimbursed somehow for my Visits?
Yes! We can provide our patients with a superbill after each pay-per-visit appointment. A superbill is a detailed receipt that patients can submit to their insurance to receive reimbursement. However, it is important to keep in mind that not all insurance plans offer out-of-network benefits.
How do I submit a Superbill to my Insurance?
Along with the superbill, patients will also be emailed a form to fill out. They would both either have to be mailed or faxed to the designated insurance company. Please dial the phone number on the back of your plan card for member services to confirm the mailing address and fax number.
What can I use my Insurance for?
Labs, imaging, and medications will continue to be billed through insurance just like any other traditional primary care practice.
What are the New Costs for Concierge Care?
I.M. Well Medical will be offering Unlimited Concierge Primary Care at a fee of $12,000 annually, with monthly, quarterly, or bi-annual billing cycles available. The fee is reduced to $10,000 annually if payment is made up-front.
As a Concierge Patient, you will have:
Direct Access:
Providing the Physician’s cell phone number for direct and immediate communication.
After-Hours Care:
Availability beyond regular office hours for urgent medical needs.
Emergency Support:
Meeting you at the emergency department to be present during critical situations.
Inpatient Care:
Acting as your Physician if you require hospital admission, ensuring continuity of care.
Flexible Scheduling:
Unlimited rescheduling and cancellations, along with after-hours scheduling, offering significant convenience.
